Both Column Fracture of the Acetabulum Operated Using T riradiate Transtrochanteric Approach and Arthrotomy of the Hip Joint

Abstract
Purpose: The purpose of current study was to describe the results of complex acetabular fractures treated with open reduction using a transtrochanteric approach and arthrotomy of the hip joint.

Materials and Methods: Twelve consecutive patients with both column fractures of the acetabulum were treated with open reduction and internal fixation. The acetabulum was approached with a Y shaped triradiate incision, osteotomy of the greater trochanter, and arthrotomy of the hip joint. The fracture was fixed with reconstruction plates and with plates and wires in six patients, respectively. Anatomical reduction was achieved in seven patients.

Results: All the patients were followed for an average of 3.8 years. All the fractures and all osteotomies were united by the time of the last follow-up. Three patients had myositis ossificans and three patients had arthritic changes of the hip joint. No femoral head necrosis was observed. Ten patients had excellent or good outcomes in clinical score at the last follow-up. None of the patients underwent total hip arthroplasty.

Conclusion: This extensile approach allowed a good exposure of the fracture site, easy removal of osteochondral fragments, more accurate reduction, easier fixation of fracture fragments, and good or excellent clinical outcome at mid-term follow-up.
